Spayed / Neutered

If your dog is spayed/neutered, we will need documentation from your veterinarian that the dog has been sterilized.

Rabies Vaccination Certificate

During the application process, you will be provided with an opportunity to scan and submit your completed Rabies Vaccination Certificate (RVC).

** During the online renewal process, you will be required to provide information on your pet's current rabies vaccination. There is an option to upload a copy of the rabies vaccination certificate, however the system will only take image files (jpg, png, gif, bmp, or tiff). It cannot accept PDF files at this time.
