Death Certificates


We are OPEN for walk-in service for issuance. However, we are only offering appointments for all requests to change or update an AZ birth or death certificate or register a Home Birth.

Three Ways to Apply

In Person

We have 4 locations across the valley to serve you Monday-Friday from 8:00am-4:30pm. Same day service for eligible, complete applications!  

Tip! Be sure to bring your current, signed, photo ID, fee and check that you are lawfully eligible to apply.  Please see below.

Mail

Complete, signed applications with all required fees and information are usually processed within one (1) week. 

Tip! Attach a photocopy (front and back) of your current, signed, photo ID. Attach any additional required forms or documentation and submit with your signed application and fee.

Online

Online ordering through a separate company, VitalChek is available. Additional processing fees and charges for expedited service in addition to the fee per certified copy ordered. All the rules and requirements for in person and mail applications apply.

No matter which way you choose to apply, there are 3 main steps:

Step 1

Check to ensure the person applying is eligible to apply under state law

Step 2

Complete and sign the application. Include photocopies of any supportive documentation needed to establish eligibility.

Step3

Pay the appropriate fee

Note: 

  • All documents submitted will be reviewed and verified before we are able to fulfill the service request.
  • All non-English language documents submitted as evidentiary supporting documentation must be accompanied by a certified translation, please. No exceptions. Thank you.

What Years are Death Records Available?

Our office can issue ALL Certified Copies of Arizona Death Certificates. In our state, Fetal Death and Certificates of Birth Resulting in Stillbirth became available after January 1, 2013 and we can assist you with certified copies of these as well.
